Access to information (ATI) – Personal Requests
You can request records under the Access to Information Act (ATIA) for the following Government of Alberta departments through eServices: Primary and Preventative Health Services; Hospital and Surgical Health Services; and Mental Health and Addiction.
The program areas for these departments are shown in the following table.

To submit an ATI request via eServices, you will need an Alberta.ca Account to ensure your privacy is protected.
Important: Do not submit requests for "Health Information" through this site. For personal health access requests please proceed to Alberta Netcare EHR.

The Access to Information Act (ATIA) provides a method to request records held by public bodies that is not available by other means. A Personal Access Request will allow you to make a single request to a Government of Alberta department. If you want to make a request to another public body (such as a municipality or a police service), please contact the appropriate Access to Information (ATI) Office.
Personal Access Request
To determine if you need to make a request or for help completing the form, contact the ATI Office of the public body to which you are making the request.
A Personal Access Request is a request for access to records that are not the personal information of the applicant. If you are requesting access to records containing personal information about yourself or an individual you are authorized to represent, please submit a General Access Request.
There is no initial fee to submit a personal access request, but fees may apply for providing the requested records.

Submitting a Personal Access Request
- Send any documents supporting your request to the GoA department ATI Office identified in your completed ATI request.
- You will be required to provide prrof of your identity before records containing your personal information are released to you.
- The transaction receipt for this ATI request indicates that your request has been submitted, but it does not indicate that the appropriate ATI Office has received the request.
- A request is considered received when it is opened at the department ATI Office authorized to receive it during standard office hours.
- Standard Government of Alberta business hours are from 8:15 a.m. to 4:30 p.m. Monday to Friday (excluding statutory and other designated holidays).
Associated Costs
- There is no initial fee to submit a personal access request, but fees may apply for providing the requested records.
- If the total cost of producing a copy of the records is more than $10, you may be charged a fee for the copy. The ATI office will advise you if any fees apply.
- The records are provided when the fee is paid in full.
Health Information Requests

Albertans can sign up for MyHealth Records to access some of their health information online such as lab results, medications, and immunizations drawn from Alberta Netcare. Please note that this is not an all inclusive health medical record and does not replace the legal record of any provider.

Select your registry agent. Registry agents offer registration, information and licensing services on behalf of government. The Government of Alberta is working collaboratively with registry agents to bring more government services online. You are not required to visit the selected registry agent.
