Court of King's Bench Jury Expense Request
Use this form to submit a claim for eligible expenses and allowances for attending Jury Selection.
If you were selected to sit as a juror on a jury trial, do not use this form. Read the Eligibility Criteria before proceeding.

Eligibility Criteria
- You must have attended jury selection in response to a jury summons.
- Reimbursement is eligible for expenses and allowances under s. 4(1)(c) of the Jury Act Regulation, as supplemented by the Travel, Meal and Hospitality Expenses Policy.
- The daily fee outlined in the Jury Act Regulation under s. 4(1)(b) does not apply to individuals who are not selected as jurors.
- Individuals cannot claim expenses and allowances if their jury selection has been cancelled.
Individuals selected to serve on a jury trial are not eligible to submit a claim through this process. Instead, jurors should complete the specific reimbursement forms provided by the Jury Officer at the time of their service. These forms are tailored for jury-related expenses and ensure proper processing in accordance with applicable policies and procedures. If you have any questions or need assistance with completing the forms, please contact the Jury Officer directly for guidance.
All requests must be submitted within 60 days of the jury selection date. Late submissions may not be considered.
The following are not reimbursable:
- lost wages or personal income;
- non-essential or excessive costs (e.g., valet parking); and
- any other expenses not allowed in the Jury Act Regulation.
Claims must be submitted through the designated digital service or via an alternate method approved by Court Operations.
All claims will be reviewed for accuracy and compliance with the policy. Incomplete or inaccurate submissions may be returned or denied.
Approved reimbursements will be processed and mailed within 45 days of submission.

Additional Notes:
- It is the responsibility of the claimant to keep receipts for expenses claimed for one year.
- Submitting a false claim may result in denial of reimbursement and could lead to legal consequences.
