If you were involved in a traffic collision in Alberta, you may request a copy of the Alberta Collision Report.

Before ordering please contact ecollisionadmin@gov.ab.ca with name and police file number to enquire if Alberta Transportation has received the collision report from the enforcement agency.

 

Alberta Transportation does not provide any witness statements or photographs taken by enforcement, or whether charges have been laid. If you require these items with the police copy, you will need to contact that enforcement agency.

This authorization shall be valid for a period of one (1) year from the date hereof.

Details

The following is required:

  • If you are acting on behalf of an individual involved in a collision in Alberta, you must provide a
    signed authorization from that person allowing release of information relative to the specific
    collision occurrence, to you. Please ensure that their full name is printed or typed on the form,
    along with their signature.
  • The position in vehicle (i.e. driver, passenger, pedestrian, etc.) of the person for whom you are
    requesting information.
  • A payment made to the Government of Alberta (Canadian funds only) in the amount of
    $15.75 (15.00 + $0.75 GST) for each form requested. If no form is located, the payment will be
    returned.
  •  

Please note that Alberta Transportation may not receive the original Alberta Collision Report Form for
several months after the date of the collision. As well, Alberta Transportation does not receive collision
statements (involved party or witness) or any other collision information beyond the Alberta Collision
Report Form.
The original Alberta Collision Report Form is kept on file for a maximum of 10 years and is then
destroyed. Copies of fatal collisions are kept for a maximum of 25 years.
